School procedures for administering medication in school.
If your child has a long term medical condition that requires the long-term administration of medication, e.g. asthma, diabetes etc, then a care plan must be completed with Mrs Laider or Mrs Guy. An appointment should be arranged via the school office.
Occasionally children require prescribed, short-term medication to be administered in school. We require written consent from parents/carers in line with school policy on the administration of medication at Etherley Lane. If you child requires prescribed, short-term medication to be administered, please call in to the office to sign the relevant form.
Alternatively please download and sign the form below then return it to the school office with your child’s medication.
Please note that only prescribed medication can be administered and it MUST be labelled by a doctor or pharmacist and be in the original container.
